Your first visit
Please bring:
- Photo ID and your insurance card
- A list of your current medications
- The shoes you wear most often
- Any recent X-rays or test results for your feet
New patient forms
Please arrive about 15 minutes early for your first visit to complete new-patient paperwork.

Have a question about your insurance or coverage? Call any of our offices and we'll be happy to help.
House calls
For patients who are unable to travel, house calls are available. Call your nearest office to arrange a visit.
Ready to get back on your feet?
Call the office nearest you. We're often able to see new patients the same day.
